Run Cairns Example again and update the manual.

Description
Discrepancies between the results reported in the manual and actual runs with recent versions of ANUGA have been found (see http://sourceforge.net/mailarchive/forum.php?thread_name=d7c6ed480710292319t535cf214ne771d74c6d143e58%40mail.gmail.com&forum_name=anuga-user and http://sourceforge.net/forum/forum.php?thread_id=1861165&forum_id=593114 as well as attachments mailed directly to us by Rajaraman).
One thing we could do is to rerun the Cairns example and verify the outputs are consistent with the manual.
